Résumé

La trachéotomie pédiatrique entraîne des modifications anatomiques et fonctionnelles majeures, impactant la respiration, la phonation, la déglutition et l’oralité alimentaire. Ces changements peuvent être difficiles à appréhender pour les parents comme pour les professionnels. Peu d’études s’y sont intéressées et peu d’outils les illustrent. Notre étude avait pour objectif d’identifier les besoins d’information des orthophonistes et des parents d’enfants trachéotomisés, en vue de créer un outil pédagogique facilitant la compréhension de la trachéotomie et ses implications. Pour ce faire, 95 orthophonistes et 38 parents ont répondu à un questionnaire en ligne. L’analyse des données a mis en évidence un besoin de clarification des conséquences anatomiques et fonctionnelles de la trachéotomie, ainsi qu’un intérêt à l'élaboration d’un outil appuyant les explications verbales de l’orthophoniste. Les parents ont exprimé un quotidien angoissant, stressant et fatigant, associé à un bouleversement familial, social et professionnel. Une meilleure compréhension de la trachéotomie pourrait les aider à appréhender plus sereinement ce quotidien. Ainsi, un mannequin pédiatrique trachéotomisé avec coupe sagittale a été conçu en collaboration avec des étudiants ingénieurs, au moyen d’imprimantes 3D et accompagné d’un livret d’utilisation. Notre étude souligne l’importance de l’accompagnement parental dans la prise en soin des enfants trachéotomisés. Elle ouvre la voie à de futures recherches portant sur le vécu des enfants trachéotomisés et leur inclusion scolaire.

Pediatric tracheotomy leads to significant anatomical and functional changes, affecting breathing, phonation, swallowing, and feeding. These changes can be difficult for parents and professionals to understand. Few studies have focused on this topic and few tools illustrate it. Our study aimed to identify the informational needs of speech and language therapists and parents of tracheotomized children, with the aim of creating an educational tool to improve understanding of tracheotomy and its implications. To this end, 95 speech and language therapists and 38 parents responded to an online questionnaire. Data analysis revealed a need for clarification of the anatomical and functional consequences of tracheotomy, as well as an interest in developing a visual tool to support the verbal explanations of the speech and language therapist. Parents expressed a distressing, stressful, and tiring daily life, associated with family, social, and professional upheaval. A better understanding of tracheotomy could help them to approach this daily life more serenely. In response, a pediatric tracheotomized model with a sagittal section was designed in collaboration with engineering students, using 3D printers and accompanied by a user guide. Our study highlights the importance of parental support in the care of tracheotomized children. It paves the way for future research on the experiences of tracheotomized children and their school inclusion.
