<dcterms:abstract xml:lang="fr">Contexte : Le traumatisme de l’épaule est un motif de consultation fréquent. L’examen clinique
initial peut être compliqué à cause de la douleur et ne permet pas toujours d’établir un
diagnostic. Il est donc primordial d’effectuer une réévaluation à distance du traumatisme qui
permettra d’affiner le diagnostic et d’adapter la prise en charge. L’objectif principal de ce travail
est d’évaluer la prise en charge en médecine de ville des patients avec un traumatisme de
l'épaule non grave et de proposer des pistes d’amélioration.
Méthode : Il s’agit d’une étude monocentrique, descriptive, rétrospective avec inclusion
prospective réalisée du 1er juillet au 31 décembre 2023. Tous les patients âgés de plus de 15
ans et 3 mois ayant présenté un traumatisme de l’épaule pour lequel ils ont consulté aux
urgences du CH A. Lepève de Dunkerque étaient éligibles. Ils ont reçu un appel téléphonique 6
semaines après leur passage aux urgences. Cela permettait de leur soumettre un
questionnaire. Les données étaient complétées par l’étude du dossier médical informatique.
Une déclaration à la CNIL a été effectuée.
Résultats : Sur les 185 patients éligibles, 151 ont été inclus. 53,8% des patients ont bénéficié
d’une consultation médicale dans les suites de leur traumatisme de l’épaule et le délai moyen
de consultation était de 13 jours.
10,6% des patients ont bénéficié d’une ordonnance d’échographie aux urgences alors que les
médecins généralistes en ont prescrit dans 26,7% des cas. 6,6% des patients ont bénéficié
d’ordonnance de séances de kinésithérapie aux urgences alors que les médecins généralistes
en ont prescrit dans 34,8% des cas.
Sur les 107 contusions diagnostiquées aux urgences, seules 33 (30,8%) étaient effectivement
des contusions.
Au total, 101 (66,9%) patients présentaient des symptômes résiduels. Les patients ayant des
symptômes résiduels ont consulté en moyenne au bout de 15 jours alors que ceux n’ayant pas
de symptôme résiduel ont consulté en moyenne au bout de 9 jours.
La durée moyenne d’immobilisation était de 20 jours. Entre 85 et 100% des patients ont réalisé
les examens prescrits.
Conclusion : La consultation de contrôle par le médecin généraliste permet une plus grande
prescription d’examens complémentaires permettant d’affiner les diagnostics finaux et ainsi
d’optimiser la prise en charge. Cependant 41,7% des patients ne reconsultent pas. La création
d’une consultation post-urgence pourrait permettre une amélioration du parcours de soins des
patients.</dcterms:abstract>
<dcterms:abstract xml:lang="en">Context: Shoulder injuries are a common presentation to general practice. Clinical examination
may be difficult due to pain levels during the acute phase, and is not always sufficient to
formulate a diagnosis. Therefore, it is essential to reassess the injury at a later time in order to
refine the diagnosis and to adapt patient management accordingly. The main objective is to
assess patient management in the general practice setting for non-severe shoulder injuries and
to offer ways to standardise practices.
Method: This is a retrospective study with prospective inclusion carried out from July 1 to
December 31, 2023. All patients aged over 15 years and 3 months who presented with a
shoulder injury at the emergency department of Dunkirk Hospital were eligible. They received a
telephone call 6 weeks after their discharge. This allowed the submission of a questionnaire to
them. Data collection was supplemented by a study of their computerized medical record.
Results: Out of 185 eligible patients, 151 were included. 53.8% of the patients had a general
practice appointment scheduled following up to their visit to the emergency department. The
average time between the hospital discharge and the appointment was of 13 days.
10.6% of patient were prescribed an ultrasound at the emergency department, while general
practitioners prescribed an ultrasound for 26.7% of the patients. 6.6% of the patients were
prescribed physical therapy at the emergency department, while general practitioners
prescribed physical therapy to 34.8% of the patients.
Out of 107 contusions diagnosed at the emergency department, only 33 (30.8%) were indeed
contusions.
In total, 101 patients (66.9%) experienced residual symptoms. Patients experiencing residual
symptoms presented on average 15 days after their hospital discharge, whereas patients not
experiencing residual symptoms presented on average 9 days after their discharge.
The average immobilisation period was of 20 days. Between 85 and 100% of the tests ordered
were performed.
Conclusion: The follow-up appointment with the general practitioner leads to the prescription of
additional tests and to further investigation, allowing for a sharper final diagnosis and thus
improving case management. Nonetheless, 41.7% of patients do not present for a follow-up
appointment. Creating a post-discharge appointment might lead to an improved care pathway
for the patients.</dcterms:abstract>
