Par ailleurs, notre équipe a mis en évidence une expansion crânienne postérieure sur plusieurs patients ayant bénéficié d’une distraction médiofaciale sans ostéotomie combinée à un avancement fronto-orbitaire. Objectifs : Le but de cette étude est d’évaluer les volumes crânien et orbitaire et l’avancée du massif facial après ostéotomies de Le Fort III et distraction médiofaciale chez des patients atteints de craniofaciosténoses syndromiques. Patients et méthodes : Chez 8 patients, les scanners pré- et post-distraction ont été analysés selon une orientation vestibulaire. Les trois fosses crâniennes et les orbites droite et gauche ont été segmentées coupe par coupe puis leurs volumes calculés. Pour évaluer l’avancée du massif facial, différents segments et angles entre plusieurs points anatomiques (point A, basion, épines nasales antérieure et postérieure, nasion et selle turcique) ont été calculés. Un test des rangs signés de Wilcoxon pour série appariée a été utilisé pour comparer les mesures avant et après distraction. Le risque de première espèce alpha a été fixé à 5% pour toutes les analyses. Résultats : Les volumes orbitaires ont significativement augmenté, de 37,7% pour le côté gauche et de 35,2% pour le côté droit. Une augmentation moyenne significative du volume crânien total a été retrouvée (+ 9,6%). Une avancée et une augmentation de hauteur significatives du massif facial ont été retrouvées avec une augmentation des longueurs S – ENP (+ 36,9%), Ba – ENP (+ 29,2%), N – ENA (+ 24,5%) et des angles S-N-ENP (+ 36%) et S-N-A (+ 14,1%). Enfin, chez 6 patients, l’angle N-S-ENA a augmenté, traduisant une rotation horaire du segment facial distracté, et chez 2 patients, cet angle s’est fermé traduisant une rotation antihoraire du segment facial distracté. Conclusion : La distraction médiofaciale après ostéotomies de Le Fort III permet une augmentation significative des volumes orbitaire et crânien ainsi qu’une avancée significative du massif facial. Background : LeFort (LF) III midface distraction osteogenesis is one of the main techniques for correcting midface deficiency encountered in craniofaciosynostosis. Our team has recently demonstrated posterior cranial expansion in several patients who have undergone midface distraction without osteotomy combined with fronto-orbital advancement. Objectives : The aim of this study was to evaluate the cranial and orbital volumes and the advancement of the LeFort III segment following distraction in patients with syndromic craniofaciosynostosis. Patients and methods : In 8 patients, computed tomography scans before and after midface distraction were analysed in vestibular orientation. The three cranial fossae and the right and left orbits were segmented slice by slice and their volumes calculated. To assess the advancement of the LF III segment, several lengths and angles between anatomical points (point A, basion, anterior and posterior nasal spines, nasion and sella turcica) were calculated. The Wilcoxon signed rank test was used to compare the measured values before and after midface distraction. A p-value of less than 0.05 was considered to be statistically significant. Results : After LF III advancement, orbital volumes increased significantly, by 37.7% on the left side and 35.2% on the right side. There was a significant average increase in total cranial volume (+9.6%). A significant advance and increase in height of the LF III segment was found, with an increase in the lengths S - PNS (+ 36.9%), Ba - PNS (+ 29.2%), N - ANS (+ 24.5%) and the angles S-N-PNS (+ 36%) and S-N-A (+ 14.1%). Finally, in 6 patients, the N-S-ENA angle increased, reflecting a clockwise rotation of the distracted LF segment, and in 2 patients, this angle closed, reflecting a counter-clockwise rotation of the distracted facial segment. Conclusion : LF III midface distraction in patients with syndromic craniofaciosynostosis allows a significant increase in orbital and cranial volumes, as well as a significant advancement of the middle third of the face.
