<dcterms:abstract xml:lang="fr">Objectif. L’endocardite infectieuse est une pathologie grave dont l’incidence ne cesse de s’accroitre surtout en population âgée avec un pronostic défavorable. Sa prise en charge en population âgée est devenue un défi. L’objectif principal de cette étude était de mesurer la conformité globale du traitement antibiotique de l’endocardite infectieuse sur base des recommandations de l’ESC 2015 dans une population âgée de 75 ans et plus hospitalisée avec un diagnostic d’endocardite infectieuse.
Méthodes. Nous avons réalisé une étude observationnelle longitudinale rétrospective multicentrique incluant les patients âgés de 75 ans ou plus avec un diagnostic principal d’endocardite infectieuse ou de bactériémies à staphylocoque, streptocoque, entérocoque ou BGN dans le Centre Hospitalier et Universitaire de Lille et le Centre Hospitalier de Valenciennes.
Résultats. 122 patients ont été inclus dans cette étude sur résultats partiels. L’âge moyen était de 84.6 ± 5.3 ans. Tous les patients ont bénéficié d’une antibiothérapie pour l’EI et 75.4% des patients étaient sous antibiothérapie probabiliste avant l’identification du germe (78.2% dans le sous-groupe « traitement non-conforme » et 70.5% dans le sous-groupe « traitement conforme » (p=0.34)). Concernant la 1ère ligne de traitement, 42.6% des patients ont poursuivi le traitement jusqu’à la date d’arrêt programmée sans différence selon le sous-groupe de conformité (42.3% dans le sous-groupe « traitement non-conforme » versus 43.2% dans le sous-groupe « traitement conforme » (p=0.93)). Dans la population totale, 24.6% (n= 30) des patients ont présenté un évènement indésirable justifiant l’arrêt prématuré de la 1ère ligne d’antibiotique prescrite. A l’issue du séjour, 87.8% des patients présentaient une perte d’autonomie aux ADL et 62.6% des patients n’ont pas pu regagner leur lieu de vie habituel à la sortie. Sur les 41 patients présentant une indication chirurgicale théorique, 6 (14.6%) ont été opérés. Concernant l’objectif principal sur la conformité du traitement antibiotique, sur les 122 patients inclus, 78 (soit 63.9%) ont reçu un « traitement globalement non conforme » (traitement non conforme pour la 1ère ligne d’antibiotique prescrite et/ou pour la durée de traitement complète) et 44 (soit 36.1%) ont reçu un « traitement globalement conforme » (traitement conforme pour la 1ère ligne d’antibiotique prescrite et pour la durée de traitement complète) selon les recommandations de l’ESC 2015. La durée totale de traitement était de 5,3±2,8 semaines dans la population totale de l’étude et était statiquement plus courte dans le sous-groupe « traitement globalement non-conforme » (4,7±2,3 versus 6,6±3,1 semaines (p=0,001)). L’analyse de la survie moyenne des deux sous-groupes de patients n’a pas mis en évidence de différence statistiquement significative entre le sous-groupe « traitement globalement conforme » et le sous-groupe « traitement globalement non-conforme » (hazard ratio 1.63 (IC 95% 0.72-3.69)) (p =0.24).
Conclusion. L’objectif principal de ce travail était d’objectiver l’absence de conformité dans la prise en charge antibiotique de l’endocardite infectieuse chez la population âgée selon les recommandations européennes de 2015. Ces résultats incitent à la révision des directives européennes actuelles et la réflexion sur des stratégies thérapeutiques adaptées aux spécificités des sujets âgés.</dcterms:abstract>
<dcterms:abstract xml:lang="en">Objective. Infective endocarditis is a serious disease whose incidence is constantly increasing, especially in the elderly population, with a poor prognosis. Its management in the elderly population has become a challenge. The main objective of this study was to measure overall compliance with antibiotic treatment of infective endocarditis based on the ESC 2015 recommendations in an elderly population aged 75 years and over hospitalised with a diagnosis of infective endocarditis.
Methods. We carried out a multicentre retrospective longitudinal observational study including patients aged 75 or over with a principal diagnosis of infective endocarditis or bacteremia due to staphylococcus, streptococcus, enterococcus or BGN in the Lille University Hospital and the Valenciennes Hospital.
Results. 122 patients were included in this partial-results study. The mean age was 84.6 ± 5.3 years. All patients received antibiotic therapy for the IE and 75.4% of patients were on probabilistic antibiotic therapy before the identification of the germ (78.2% in the "non-compliant treatment" subgroup and 70.5% in the "compliant treatment" subgroup (p=0.34)). Regarding the 1st line of treatment, 42.6% of patients continued treatment until the programmed stop date, with no difference according to compliance subgroup (42.3% in the "non-compliant treatment" subgroup versus 43.2% in the "compliant treatment" subgroup (p=0.93)). In the total population, 24.6% (n=30) of patients presented an adverse event justifying premature discontinuation of the 1st line of antibiotics prescribed. At the end of the stay, 87.8% of patients showed a loss of autonomy in terms of ADL and 62.6% of patients were unable to return to their usual place of residence at discharge. Of the 41 patients with a theoretical indication for surgery, 6 (14.6%) were operated on. Regarding the primary objective on antibiotic treatment compliance, out of the 122 patients included, 78 (i.e. 63.9%) received "globally non-compliant treatment" (non-compliant treatment for the 1st line of antibiotic prescribed and/or for the full treatment duration) and 44 (i.e. 36.1%) received "globally compliant treatment" (compliant treatment for the 1st line of antibiotic prescribed and for the full treatment duration) according to the ESC 2015 recommendations. The total duration of treatment was 5.3±2.8 weeks in the total study population and was statically shorter in the "globally non-compliant treatment" subgroup (4.7±2.3 versus 6.6±3.1 weeks (p=0.001)). Analysis of the mean survival of the two patient subgroups showed no statistically significant difference between the "globally compliant treatment" subgroup and the "globally non-compliant treatment" subgroup (hazard ratio 1.63 (95% CI 0.72-3.69)) (p=0.24).
Conclusions. The main aim of this study was to identify the lack of compliance in the antibiotic management of infective endocarditis in the elderly population according to the 2015 European recommendations. These results suggest that the current European guidelines should be revised and that thought should be given to therapeutic strategies adapted to the specific characteristics of elderly subjects.</dcterms:abstract>
