Démence fronto-temporale et cognition sociale : élaboration d’un entraînement à la reconnaissance des émotions faciales Hadj Larbi, Louisa Démence fronto-temporale ; cognition sociale ; émotions faciales ; orthophonie Frontotemporal dementia ; social cognition ; facial emotions ; speech therapy La Démence Fronto-Temporale de type comportemental (DFTc) fait partie des dégénérescences lobaires fronto-temporales qui, après la maladie d’Alzheimer, sont la deuxième cause de démence du patient jeune. Cette pathologie se traduit, entre autre, par une perturbation de la cognition sociale dont les symptômes s’apparentent à ceux rencontrés dans le champ de la psychiatrie. L’un d’entre eux est un trouble de la reconnaissance des émotions faciales (REF), qui induit des troubles du comportement dont le retentissement sur les patients, mais aussi et surtout sur leurs aidants est très important. Dans le cadre de ce travail, nous avons souhaité élaborer un protocole d’entraînement à la REF destiné à des patients atteints de DFTc. Notre objectif étant d’obtenir, par une amélioration de la REF, une amélioration des compétences communicationnelles des sujets entraînés. Pour cela, nous nous sommes inspirés des programmes de remédiation cognitive visant la cognition sociale. Nous avons, en outre, utilisé un support informatique destiné aux orthophonistes, le logiciel Com’Emotion, qui nous a permis d’élaborer un entraînement analytique, moteur et sémantique de la REF. The Behavioural Variant of Fronto-Temporal Dementia (bvFTD) is one of the frontotemporal lobar degenerations which are, after Alzheimer's disease, the second most common cause of dementia in young patients. This pathology, among other things, is reflected in a disturbance of social cognition whose symptoms are similar to those encountered in the field of psychiatry. One of them is a disorder of facial emotion recognition (FER), which has a strong impact on patients, but also and especially on their caregivers. In the context of this work, we wanted to develop a FER training protocol for patients with bvFTD. Our goal was to obtain, through an improvement of the FER, an improvement of the communication skills of the trained participants. For this purpose, we drew inspiration from cognitive remediation programs targeting social cognition. In addition, we used a software designed for speech therapists, Com’Emotion, which allowed us to develop an analytical, motor and semantic training of the FER.
