<dcterms:abstract xml:lang="fr">Contexte : L’ingestion de produits caustiques est associée à une mortalité faible
mais une morbidité élevée. Il n’existe pas de recommandation concernant les
modalités du support nutritionnel au cours de cette intoxication pour les stades
initiaux « intermédiaire » ZARGAR 2A. L’objectif de ce travail est de comparer le
pronostic et la survenue de complications chez des patients avec atteinte digestive
(ZARGAR 2A) selon la précocité du support nutritionnel entéral.
Méthode : Il s’agit d’une étude observationnelle, rétrospective, monocentrique
réalisée entre janvier 2013 et avril 2022 dans le pôle de Médecine Intensive
Réanimation du centre hospitalier universitaire de Lille. Les données
démographiques, cliniques, biologiques et endoscopiques à l’admission et durant
l’hospitalisation ont été collectées de façon anonyme. Les patients ont été classés en
2 groupes (« précoce et « tardif ») selon la précocité du support nutritionnel entéral.
Nous avons élaboré un critère de jugement principal composite, regroupant la
survenue d’au moins une complication intra-hospitalière (sténose digestive,
thrombose veineuse, infection, décès et chirurgie). Les objectifs secondaires sont de
comparer la survenue de ces complications de façon isolée, les durées
d’hospitalisation, d’analyser les facteurs de risque d’un défaut de cicatrisation et de
décrire la prise en charge hospitalière.
Résultats : Soixante patients ont été inclus dans l’étude avec 24 patients classés
dans le groupe nutrition entérale « précoce » et 36 patients dans le groupe nutrition
entérale « tardive ». La survenue d’au moins une complication a été retrouvée chez
28% des patients (n=17). L’étude comparative entre les 2 groupes a montré une
différence statistiquement significative avec 14 évènements dans le groupe « tardif »
et 3 dans le groupe « précoce » (p=0,03). Les évènements infectieux étaient plus
fréquents dans le groupe « tardif » (p=0,01) et 2 épisodes de thrombose veineuse
ont été retrouvés uniquement dans ce groupe. La durée d’hospitalisation était
significativement plus courte chez les patients du groupe nutrition entérale
« précoce » (p=0,01). Un défaut de cicatrisation endoscopique était retrouvé chez 9
patients (15%) sans différence entre les groupes. L’analyse n’a pas permis de
retrouver de facteur de risque même après ajustement sur le délai de réalisation de
la FOGD de réévaluation.
Conclusion : Notre étude souligne la supériorité d’un support nutritionnel entéral
« précoce » qui permet une diminution des complications infectieuses et
thrombotiques ainsi qu’une diminution de la durée d’hospitalisation.</dcterms:abstract>
<dcterms:abstract xml:lang="en">Introduction : Caustic ingestion is associated with low mortality but high morbidity.
There are no recommendations regarding nutritional support during this intoxication
for the initial "intermediate" stages ZARGAR 2A. The objective of this study is to
compare the prognosis and the occurrence of complications in patients with digestive
damage (ZARGAR 2A) according to the early stage of enteral nutritional support.
Method : This is an observational, retrospective, monocentric study conducted
between January 2013 and April 2022 in the intensive care unit of the Lille University
Hospital. Demographic, clinical, biological, and endoscopic data at admission and
during follow-up were collected anonymously. Patients were classified into 2 groups
("early" and "late") according to the earliness of enteral nutritional support. We
developed a composite primary endpoint, grouping the occurrence of at least one
intra-hospital complication (digestive stenosis, venous thrombosis, infection, death
and surgery). The secondary endpoints were to compare the occurrence of these
isolate complications, the length of hospitalization to highlight the risk factors of a
healing defect.
Results : Sixty patients were included with 24 patients classified in the "early" enteral
nutrition group and 36 patients in the "late" group. The primary composite endpoint
occurred in 28% of patients (n=17). The comparative study between the 2 groups
showed a statistically significant difference with 14 events in the "late" group and 3 in
the "early" group (p=0.03). Infectious events were more frequent in the "late" group
(p=0.01). Two episodes of venous thrombosis were found in the "late" group. The
length of hospitalization was significantly shorter in the "early" enteral nutrition group
(p=0.01). Endoscopic healing defect was found in 9 patients (15%) with no difference
between the groups. The analysis did not reveal any risk factor, even after
adjustment of the delay to perform endoscopic follow-up.
Conclusion : Our study confirms the superiority of "early" enteral nutrition with a
decrease in infectious and thrombotic complications as well as a decrease in
hospitalization time.</dcterms:abstract>
