L’éthanol est un toxique communément consommé et dont l’abus chronique est associé à de nombreux problèmes sociétaux et pathologies. Les biomarqueurs directs actuels d’une consommation d’alcool ne permettent de détecter et d’estimer cette consommation que durant 24 heures pour l’éthanol et 3 jours pour l’éthylglucuronide urinaire. Ainsi, nous avons cherché à identifier un biomarqueur direct permettant d’estimer la consommation d’alcool sur une période plus importante. Pour cela, nous avons développé une méthode de dosage du phosphatidyléthanol dans le sang séché par chromatographie liquide couplée à un spectromètre de masse en tandem. En effet, ce nouveau biomarqueur offre une fenêtre de détection de 21 à 28 jours. Nous avons développé cette méthode de dosage en optimisant les paramètres pré-analytiques (le moyen de prélèvement, la stabilité et l’extraction du phosphatidyléthanol), puis analytiques (séparation chromatographique et paramètres de détection spectrométrique). Le développement de cette méthode se base notamment sur les résultats de stabilité pré-analytique qui nous ont permis de définir une méthode de conservation applicable en routine (prélèvement sur buvards). Cette méthode de dosage a été validée et accréditée selon la norme EN ISO 15189 afin de la proposer en routine dans notre laboratoire. Enfin, nous avons entrepris le développement d’essais cliniques avec les cliniciens des principaux domaines médicaux intéressés tels que l’addictologie, la transplantation et le suivi de grossesse. En conclusion, ce projet a permis de développer une méthode de détection et de quantification d’un biomarqueur disposant d’une fenêtre de détection dans le sang de 21 à 28 jours permettant d’optimiser le suivi des patients alcoolodépendants.

Ethanol is a widely consumed substance, whose chronic abuse is associated with many societal issues and pathologies. Currently used biomarkers of alcohol consumption allow a maximum detection window up to 24 hours for ethanol and 3 days for ethylglucuronide in urine. Thus, we sought to identify a direct biomarker allowing an alcohol consumption estimation over a longer period. For this, we have developed a blood phosphatidylethanol assay in Dried Blood Spot (DBS) by liquid chromatography coupled with tandem mass spectrometery (LC-MS/MS). Indeed, this new biomarker offers a detection window of 21 to 28 days. We have developed this assay method by optimizing pre-analytical (the sampling method, the stability and the extraction of phosphatidylethanol) and analytical parameters (chromatographic separation and spectrometric detection parameters). This method development is mainly based on the results of pre-analytical stability which allowed us to define a conservation method applicable routinely (with DBS sampling). This method has been validated and accredited according to the EN ISO 15189 norm in order to offer it routinely in our laboratory. Finally, we have undertaken the development of clinical trials with clinicians from the main medical fields concerned, such as addictology, transplantation and obstetric. In conclusion, this project has allowed the development of a biomarker with a detection window up to 28 days, enhancing the monitoring of alcohol-dependent patients.
