<dcterms:abstract xml:lang="fr">Les troubles de la parole sont fréquents dans la maladie de Parkinson et correspondent le plus souvent à une dysarthrie hypokinétique. Parmi eux, les altérations prosodiques occupent une place importante, mais restent encore à préciser. Elles peuvent concerner la variabilité mélodique, le débit, l'organisation temporelle de la parole ou les pauses. Les traitements médicamenteux ne les résolvant pas, les approches fondées sur le rythme constituent une piste intéressante. Les stimulations auditives rythmiques sont déjà utilisées dans la maladie de Parkinson, mais leur intérêt pour la parole demeure moins établi. L'amorçage rythmique, qui consiste à exposer un individu à une structure rythmique avant une tâche, pourrait soutenir la perception temporelle, l'anticipation et la préparation motrice. L'objectif général de cette thèse était d'examiner dans quelle mesure l'amorçage rythmique peut influencer la production de la parole, notamment dans la maladie de Parkinson, et d'interroger ces effets pour mieux comprendre les troubles prosodiques. Une première étude, sous la forme d'une revue systématique et d'une méta-analyse bayésienne, a permis de préciser les dimensions prosodiques les plus robustement altérées dans cette pathologie. Les résultats suggèrent une diminution de la variabilité de la fréquence fondamentale et une augmentation de la durée des pauses, tandis que d'autres mesures apparaissent plus hétérogènes. Les études expérimentales ont ensuite évalué les effets d'amorces rythmiques auditives régulières et irrégulières sur plusieurs dimensions de la parole. Une première étude a été menée auprès de personnes atteintes de la maladie de Parkinson, de personnes atteintes d'ataxie spinocérébelleuse et de participants contrôles. Dans les trois groupes, l'effet le plus robuste concernait le temps d'initiation de la parole, avec une facilitation après amorce régulière. Les effets observés sur le débit, les métriques rythmiques, les pauses ou la variabilité mélodique étaient plus limités, et davantage dépendants de la population ou de la condition. Ces résultats suggèrent que l'amorçage rythmique agit surtout sur la préparation temporelle globale de la parole, plutôt que sur le contrôle fin de l'ensemble des paramètres prosodiques. Une deuxième étude a été réalisée auprès de personnes sans pathologie neurologique. Elle visait à comparer différentes amorces régulières et irrégulières et à examiner si leurs effets sur la parole étaient associés aux compétences musicales et rythmiques des participants, évaluées par des tâches de synchronisation sensorimotrice. Cette étude a confirmé l'effet bénéfique des amorces régulières sur le temps d'initiation de la parole, mais les compétences rythmiques mesurées ne semblent pas prédire la sensibilité à l'amorçage. Enfin, une enquête menée auprès d'orthophonistes a interrogé l'usage clinique des interventions fondées sur la musique et le rythme. Elle met en évidence l'intérêt perçu de ces approches dans la pratique orthophonique, tout en soulignant la nécessité de mieux articuler preuves scientifiques, caractéristiques individuelles des patients, objectifs thérapeutiques et contraintes de terrain. Dans leur ensemble, ces travaux suggèrent que l'amorçage rythmique auditif peut moduler certains aspects de la production de la parole, avec un effet particulièrement crédible sur l'initiation. Ils invitent toutefois à considérer ces effets comme sélectifs, dépendants des tâches, des profils individuels et des caractéristiques des amorces. Cette thèse contribue ainsi à une meilleure compréhension des liens entre rythme, prosodie et contrôle moteur de la parole, tout en ouvrant des perspectives pour le développement d'approches orthophoniques fondées sur le rythme. Les travaux futurs devront préciser les paramètres optimaux des amorces, identifier les profils de patients les plus susceptibles d'en bénéficier et explorer les mécanismes sous-jacents à ces effets.</dcterms:abstract>
<dcterms:abstract xml:lang="en">Speech disorders are common in Parkinson's disease and most often take the form of hypokinetic dysarthria. Among these disorders, prosodic alterations play a significant role, but their precise nature remains to be clarified. They may involve melodic variability, speech rate, temporal organization of speech, or pauses. These dysarthric disorders are not resolved by standard drug treatments. In this context, rhythm-based approaches represent a particularly interesting avenue of investigation. Auditory rhythmic stimulation is already used in Parkinson's disease, particularly to support certain aspects of motor control such as walking. However, its benefits for speech remain less well established. Rhythmic priming, which involves exposing an individual to a rhythmic structure prior to performing a task, offers particularly interesting prospects in this regard. It could support temporal perception, anticipation and motor preparation, and thus modulate certain aspects of speech production. The aim of this thesis was to examine the extent to which rhythmic priming can influence speech production, particularly in people with Parkinson's disease, and to investigate the effects observed in order to better understand and manage prosodic disorders. An initial study, in the form of a systematic review and a Bayesian meta-analysis, identified the prosodic dimensions most robustly altered in Parkinson's disease. The results suggest a reduction in fundamental frequency variability and an increase in pause duration. Other measures, however, appear more heterogeneous. Experimental studies then assessed the effects of regular and irregular auditory rhythmic primes on various aspects of speech. An initial study was conducted with two groups of participants with neurodegenerative disorders —Parkinson's disease and spinocerebellar ataxia— and a control group. In these three groups, the most robust effect concerned speech initiation time, with facilitation following a regular prime. The effects observed on rate, rhythmic metrics, pauses or melodic variability were more limited, and more dependent on the population or condition. These results suggest that rhythmic priming acts primarily on the overall temporal preparation of speech, rather than on the fine and systematic control of all prosodic parameters. A second study was conducted with individuals without neurological conditions. This study sought to compare different regular and irregular priming patterns and to link the observed effect of priming on speech with the participants' musical and rhythmic skills, in particular through assessment using sensorimotor synchronization tasks. This study confirmed the beneficial effect of regular priming on speech initiation time. The rhythmic skills measured do not appear to predict the effects of rhythmic priming on speech. Finally, a survey of speech and language therapists examined the clinical use of interventions based on music and rhythm. This study highlights the perceived value of these approaches in speech and language therapy practice, whilst emphasizing the need to better align scientific evidence, patients' individual characteristics, therapeutic objectives and practical constraints. Taken together, these studies suggest that auditory rhythmic priming can modulate certain aspects of speech production, with a particularly credible effect on speech initiation time. However, they suggest that these effects should be regarded as selective, depending on the task, individual profiles and the characteristics of the primes. This thesis thus contributes to a better understanding of the links between rhythm, prosody and motor control of speech, whilst opening up prospects for the development of rhythm-based speech therapy approaches. Future research should, in particular, specify the optimal parameters of primes, identify the patient profiles most likely to benefit from them, and explore more directly the mechanisms underlying these effects.</dcterms:abstract>
