<dcterms:abstract xml:lang="fr">La peau est l'organe le plus étendu du corps humain et constitue une interface essentielle entre notre organisme et l'environnement. Elle joue un rôle clé dans la protection contre les agressions extérieures, la régulation thermique, l'immunité et les fonctions sensorielles. Son équilibre fragile dépend de nombreux facteurs, tels que l'hydratation, l'alimentation, le microbiote cutané et l'exposition au soleil. Lorsqu'il est perturbé, un état inflammatoire peut s'installer, régulièrement activé lors de dermatoses chroniques courantes comme l'acné, la dermatite atopique ou séborrhéique, le psoriasis et la rosacée.Ces pathologies résultent d'interactions complexes entre le microbiote cutané, le système immunitaire et la physiologie de la peau. Les lipides, constituants majeurs de la peau, jouent un rôle fondamental dans l'homéostasie cutanée et l'immunité, et les métabolites, essentiels à l'hydratation de la peau et aux interactions avec le microbiote, sont au cœur de cet équilibre. Leur analyse en métabolomique, a connu un essor considérable ces dernières années, notamment grâce à la spectrométrie de masse. En effet cette technologie, offre l'accès à des analyses non ciblées avec une sensibilité, une polyvalence et une reproductibilité élevées. Cependant, les méthodes conventionnelles, qu'elles soient basées sur un couplage LC-MS/MS ou sur les approches shot-gun nécessitent une préparation d'échantillon chronophages. Cette préparation d'échantillon peut induire une modification des lipides, telles que des oxydations, pouvant altérer les résultats. Par ailleurs, les analyses ex-vivo ne permettent pas de réaliser facilement des analyses et un suivi dans le temps. Il est donc essentiel de développer des approches in vivo permettant une analyse en temps réel et non invasive des lipides et métabolites cutanés.La technologie SpiderMass, développé par le laboratoire PRISM depuis plus de dix ans, est une technologie innovante utilisant une source ambiante basée sur une désorption/ionisation laser dans le moyen IR par excitation résonnante des molécules d'eau des tissus, mini-invasive et permettant analyses en temps-réel. Tout d'abord développé pour l'analyse peropératoire in vivo, dans le cadre de la chirurgie des cancers, et en particulier la définition des marges d'exérèse, la technologie SpiderMass s'est révélée être un véritable atout pour l'analyse de la peau en temps réel, de manière non invasive. Une thèse précédente a permis d'adapter cette technologie aux applications dermo-cosmétiques.Mon projet porte sur l'évaluation de l'efficacité du SpiderMass dans ce même domaine, et plus particulièrement sur l'étude des pathologies de l'épiderme. Pour cela, je me suis tout d'abord intéressée, à la caractérisation des profils moléculaires observables par SpiderMass en analysant les peaux atteintes des dermatoses inflammatoires chroniques les plus courantes afin d'identifier des biomarqueurs spécifiques. Par la suite, l'évolution de ces profils et des biomarqueurs a été étudiée après un traitement topique de la dermatite séborrhéique. L'application du SpiderMass dans plusieurs études cliniques, menées au Centre clinique Pierre Fabre Dermo-Cosmétique, a mis en évidence des différences significatives entre peau saine et pathologique, ainsi que des évolutions notables au cours des traitements. Enfin, une étude clinique sur des volontaires sains appliquant quotidiennement une crème aux rétinoïdes cosmétiques pendant huit semaines a révélé des modifications moléculaires significatives par rapport au groupe témoin utilisant un placebo.Les résultats de cette thèse démontrent le potentiel du SpiderMass pour l'étude de la physiopathologie cutanée et de son évolution, ainsi que pour l'identification de biomarqueurs d'intérêt. De plus, Cette technologie ouvre de nouvelles perspectives pour l'évaluation de l'efficacité des produits dermo-cosmétiques, notamment dans le cadre du vieillissement cutané et des maladies inflammatoires chroniques.</dcterms:abstract>
<dcterms:abstract xml:lang="en">The skin is the largest organ of the human body and serves as a critical interface between our organism and the environment. It plays a key role in protecting against external aggressions, regulating body temperature, supporting immune functions, and enabling sensory perception. Its delicate balance depends on numerous factors, such as hydration, nutrition, the skin microbiota, and sun exposure. When disrupted, an inflammatory state can arise, frequently observed in common chronic dermatoses such as acne, atopic or seborrheic dermatitis, psoriasis, and rosacea.These pathologies involve complex interactions between the skin microbiota, the immune system, and skin physiology. Among the key players in this balance, lipids and metabolites hold a central role. Lipids, as major components of the skin, play a fundamental role in skin homeostasis and immunity, while metabolites are essential for skin hydration and interactions with the microbiota. Their analysis through metabolomics has gained considerable momentum in recent years, particularly thanks to mass spectrometry. This technology enables untargeted analyses with high sensitivity, versatility, and reproducibility. However, conventional methods, whether based on LC-MS/MS coupling or shotgun approaches, require sample preparation and are time-consuming. Moreover, sample preparation itself can lead to lipid modifications, such as oxidation, which may compromise results. Additionally, ex vivo analyses do not easily allow for real-time tracking over time. Therefore, developing in vivo approaches for real-time, non-invasive analysis of skin lipids and metabolites is crucial.The SpiderMass technology, developed by the PRISM laboratory for over a decade, is an innovative ambient ionization technique based on mid-IR laser desorption/ionization through resonant excitation of tissue water molecules. This minimally invasive method allows for real-time analyses. Initially designed for intraoperative in vivo cancer surgery, particularly for defining tumor resection margins, SpiderMass has proven to be a powerful tool for real-time, non-invasive skin analysis. A previous PhD project successfully adapted this technology for dermo-cosmetic applications.My research focuses on evaluating the effectiveness of SpiderMass in dermo-cosmetics, particularly in the study of epidermal pathologies. To achieve this, I first explored the molecular profiles detectable by SpiderMass by analyzing skin affected by the most common chronic inflammatory dermatoses to identify specific biomarkers. Subsequently, the evolution of these profiles and biomarkers was studied following topical treatment for seborrheic dermatitis. The application of SpiderMass in multiple clinical studies, conducted at the Pierre Fabre Dermo-Cosmetics Clinical Center, revealed significant differences between healthy and pathological skin, as well as notable changes during treatment. Finally, a clinical study on healthy volunteers applying a cosmetic retinoid-based cream daily for eight weeks demonstrated significant molecular modifications compared to the control group using a placebo.The results of this thesis demonstrate SpiderMass's potential for studying skin pathophysiology and its evolution, as well as for identifying relevant biomarkers. This technology opens new perspectives for evaluating the efficacy of dermo-cosmetic products, particularly in the context of skin aging and chronic inflammatory diseases.</dcterms:abstract>
