<dcterms:abstract xml:lang="fr">Le diabète monogénique touche plusieurs centaines de milliers de personnes dans le monde,avec HNF1A-MODY l’une des formes les plus fréquentes. HNF1A code un facteur de transcription clé pour l’homéostasie glucidique, et ses mutations entraînent une hyperglycémie progressive caractérisée par une glycosurie rénale, une hypersensibilité aux sulfonylurées et une atteinte multi-organique. Bien que les sulfonylurées restent le traitement de première intention, leur utilisation prolongée conduit à un épuisement des cellules bêta, soulignant la nécessité de nouvelles approches thérapeutiques. Le développement de traitements est limité par l’absence de modèles précliniques pertinents. Les souris Hnf1a KO présentent un diabète précoce sévère, peu représentatif de l’évolution humaine, tandis que les modèles spécifiques aux cellules bêta n’intègrent pas l’atteinte systémique. Cette thèse décrit la création et la validation d’un nouveau modèle murin hétérozygote Hnf1a(Hnf1a+/?e4-10), conçu comme une plateforme translationnelle pour la recherche. Une caractérisation approfondie a confirmé une haploinsuffisance et une reconstitution fidèle de la maladie : progression dépendante de l’âge d’une normoglycémie vers une hyperglycémie, résistance hépatique à l’insuline, préservation de la sensibilité aux sulfonylurées, glycosurie,hyperglucagonémie, production endogène de glucose et atteinte multi-organique. Une découverte majeure a été faite : le diabète s’est développé exclusivement chez les souris mâles, révélant un lien inattendu entre la déficience en Hnf1a et la signalisation androgénique,marqué par une diminution de la testostérone et une réduction de l’expression du récepteur des androgènes dans les îlots pancréatiques. Ce mécanisme sexodépendant ouvre des perspectives cliniques importantes. Des analyses fonctionnelles ont confirmé des défauts sécrétoires multihormonaux, affectant à la fois l’insuline et le glucagon. Ce modèle validé a permis d’évaluer deux stratégies thérapeutiques innovantes : Un agoniste triple des récepteurs GLP-1/GIP/GCG, qui a montré des effets protecteurs supérieurs à ceux des sulfonylurées, préservant la sécrétion d’insuline et réduisant le stress oxydatif. Des validations sur des îlots humains ont révélé des différences interespèces : alors que les deux traitements amélioraient la fonction des îlots murins, le gliclazide induisait un stress oxydatif et une altération transcriptionnelle dans les tissus humains, tandis que l’agoniste triple préservait l’intégrité cellulaire.La protéine pancréatique PSP/reg a favorisé la régénération des cellules bêta, offrant uneapproche réparatrice plutôt que strictement sécrétagogue, avec des bénéfices thérapeutiques plus durables. L’analyse transcriptomique a mis en évidence des mécanismes distincts : les sulfonylurées activaient des voies métaboliques ciblées tout en supprimant l’inflammation, tandis que l’agoniste triple induisait une activation cellulaire plus large, incluant une réponse immunitaire accrue et une modulation de la signalisation calcique. Ce travail apporte plusieurs avancées majeures : (1) Développement du modèle préclinique le plus pertinent pour le HNF1A-MODY, (2) Identification de mécanismes pathologiques sexospécifiques, (3) Démonstration de réponses thérapeutiques spécifiques à l’espèce, avec des implications importantes pour la sécurité, (4) Validation de deux approches thérapeutiques innovantes, combinant sécrétagogues optimisés et stratégies régénératives, (5) Caractérisation moléculaire complète grâce à l’analyse transcriptomique. Ces résultats ouvrent la voie à des avancées majeures dans la prise en charge du HNF1AMODY,incluant des stratégies thérapeutiques personnalisées intégrant des biomarqueurs hormonaux sexuels, des approches pharmacologiques plus sûres, et le potentiel de thérapies combinées. Ce travail offre ainsi des perspectives inédites pour améliorer le pronostic des patients atteints de cette forme complexe de diabète monogénique.</dcterms:abstract>
<dcterms:abstract xml:lang="en">Monogenic diabetes affects hundreds of thousands globally, with HNF1A-MODY being among the most common forms. HNF1A encodes a transcription factor critical for glucose homeostasis, and mutations cause progressive hyperglycaemia with characteristic features including renal glycosuria, sulfonylurea hypersensitivity, and multi-organ dysfunction. While sulfonylureas remain first-line therapy, prolonged use leads to beta cell exhaustion,necessitating alternative treatments. Therapeutic development has been constrained by inadequate preclinical models. Complete Hnf1a knockout mice exhibit severe early-onset diabetes poorly reflecting human disease progression, while beta cell-specific models fail to capture systemic involvement. This thesis develops and validates a novel heterozygous Hnf1a mutant mouse model (Hnf1a+/?e4-10) designed as a translational research platform. Comprehensive characterisation confirmed haploinsufficiency and faithful disease recapitulation: age-dependent progression from normoglycaemia to hyperglycaemia, hepatic insulin resistance, preserved sulfonylurea sensitivity, glycosuria, hyperglucagonaemia, endogenous glucose production, and multi-organ involvement. A key discovery emerged: diabetes developed exclusively in male mice, revealing an unexpected connection between Hnf1a deficiency and androgen signalling through reduced testosterone and decreased islet androgen receptor expression. This identifies a novel sex-specific mechanism with important clinical implications. Functional studies confirmed multi-hormonal secretory defects affecting both insulin and glucagon regulation. Using this validated model, two innovative therapeutic strategies were tested: First, a GLP-1/GIP/GCG triple receptor agonist demonstrated superior protective effects compared to sulfonylureas, preserving insulin secretion and reducing oxidative stress. Critically, human islet validation revealed species-specific differences: while both treatments enhanced mouse islet function, gliclazide induced oxidative stress and transcriptional disruption in human tissue, whereas the triple agonist maintained cellular integrity. Second, pancreatic stone protein (PSP/reg) promoted beta cell regeneration, offering a reparative rather than secretagogue approach for more durable therapeutic benefits. Transcriptomic analysis revealed distinct mechanisms: sulfonylureas activated targeted metabolic pathways while suppressing inflammation, whereas triple agonist therapy induced broader cellular activation including enhanced immune responsiveness and calcium signalling. This work establishes several advances: (1) the most translationally relevant HNF1A-MODY preclinical model, (2) identification of sex-specific disease mechanisms, (3) demonstration of species-specific therapeutic responses with critical safety implications, (4) proof-of-concept for both optimised secretagogue and regenerative approaches, and (5) comprehensive molecular characterisation through transcriptomic analysis. These findings enable transformative changes in HNF1A-MODY management, including personalised treatment strategies incorporating sex hormone biomarkers, safer pharmacological approaches, and potential combination therapies. This work provides unprecedented opportunities to improve outcomes for individuals with this challenging monogenic diabetes form.</dcterms:abstract>
