Le Togo, à l'instar d'autres pays, a engagé plusieurs initiatives en ce sens, parmi lesquelles le déploiement du logiciel District Health Information Software (DHIS2), la création du Centre national de santé numérique (CNSN) ou encore la mise en place de plateformes numériques durant la pandémie. Ces actions traduisent une volonté affirmée d'améliorer l'accès aux soins et la qualité des services, tout en répondant aux recommandations de l'Organisation mondiale de la Santé (OMS) en matière d'intégration des technologies de l'information et de la communication (TIC) dans les politiques de santé. Toutefois, la numérisation soulève un défi majeur : celui de la souveraineté informationnelle et des données de santé. Le Togo demeure en effet largement dépendant d'acteurs extérieurs pour le financement, l'hébergement et la gestion de ses infrastructures numériques. Cette dépendance interroge la capacité de l'État à garantir un contrôle effectif sur la production, le traitement et la protection des données de santé. Si les sciences de l'information et de la communication (SIC) ont déjà investi le champ de la santé, elles ont encore peu exploré les enjeux de souveraineté informationnelle en santé, davantage étudiés par le droit, l'économie ou la science politique. Dans une double perspective, croisant les recherches sur la santé numérique et celles sur la souveraineté informationnelle, ce travail cherche à répondre à la question : quelles stratégies le Togo doit-il adopter pour favoriser la création d'un système d'information de santé numérique national et souverain?? L'exploration de cette problématique nous permettra de proposer une contribution au concept de souveraineté informationnelle. L'étude repose sur une enquête de terrain menée auprès des acteurs impliqués dans la numérisation. Elle s'appuie également sur une analyse comparative du traitement médiatique du concept de souveraineté dans la presse francophone, européenne et africaine. Cette recherche propose ainsi une réflexion conceptuelle et empirique sur les conditions d'émergence d'un système de santé numérique souverain, capable de répondre aux attentes des différentes parties prenantes et de soutenir durablement la politique de santé publique au Togo. Concernant la numérisation, l'étude met en évidence un certain nombre de dysfonctionnements organisationnels qui freinent l'émergence d'un SIS national. Il existe une diversité de points de vue des acteurs sur la santé numérique. Cette diversité reflète l'absence d'une stratégie nationale claire ainsi qu'un déficit de coordination et de communication entre les parties prenantes. De plus, nous avons observé une hétérogénéité des logiciels utilisés. En matière de souveraineté informationnelle, des efforts ont été réalisés, notamment avec le déploiement du DHIS2 et la construction d'un datacenter visant à territorialiser les données de santé. Toutefois, la maîtrise de la production d'information reste encore partielle. Or, cette maîtrise permettrait non seulement d'assurer un meilleur contrôle sur les données, mais aussi de produire une connaissance endogène, adaptée aux réalités locales, et donc plus pertinente pour éclairer la décision publique. Pour les acteurs, l'affirmation de la souveraineté informationnelle repose sur l'élaboration d'un cadre juridique et réglementaire, la définition d'une politique nationale de e-santé et le renforcement des infrastructures locales, afin de réduire la dépendance vis-à-vis des solutions étrangères. The healthcare sector is undergoing a profound transformation driven by digitalization, a process accelerated by the COVID-19 crisis. Togo, like other countries, has launched several initiatives in this area, including the deployment of District Health Information Software (DHIS2), the creation of the National Digital Health Center (CNSN), and the establishment of digital platforms during the pandemic. These actions reflect a clear desire to improve access to care and the quality of services, while responding to the World Health Organization's (WHO) recommendations on the integration of information and communication technologies (ICT) into health policies. However, digitization raises a major challenge: that of information sovereignty and health data. Togo remains largely dependent on external actors for the financing, hosting, and management of its digital infrastructure. This dependence calls into question the state's ability to guarantee effective control over the production, processing, and protection of health data. While information and communication sciences (ICS) have already entered the field of health, they have yet to explore the issues of informational sovereignty in health, which have been studied more extensively by law, economics, and political science. Taking a dual perspective, combining research on digital health and information sovereignty, this work seeks to answer the question: what strategies should Togo adopt to promote the creation of a national and sovereign digital health information system? Exploring this issue will enable us to contribute to the concept of information sovereignty. The study is based on a field survey conducted among actors involved in digitization. It also draws on a comparative analysis of media coverage of the concept of sovereignty in the French-speaking, European, and African press. This research thus offers a conceptual and empirical reflection on the conditions for the emergence of a sovereign digital health system capable of meeting the expectations of the various stakeholders and providing long-term support for public health policy in Togo. With regard to digitization, the study highlights a number of organizational dysfunctions that are hindering the emergence of a national HIS. There is a diversity of views among stakeholders on digital health. This diversity reflects the absence of a clear national strategy and a lack of coordination and communication between stakeholders. In addition, we observed heterogeneity in the software used. In terms of information sovereignty, efforts have been made, notably with the deployment of DHIS2 and the construction of a data center aimed at territorializing health data. However, control over information production remains partial. Yet this control would not only ensure better control over data, but also produce endogenous knowledge, adapted to local realities and therefore more relevant for informing public decision-making. For stakeholders, asserting information sovereignty relies on developing a legal and regulatory framework, defining a national e-health policy, and strengthening local infrastructure to reduce dependence on foreign solutions.
