<dcterms:abstract xml:lang="fr">Le facteur de croissance des hépatocytes (HGF) est une protéine humaine produite par les cellules stromales, qui exerce plusieurs fonctions en se liant et en activant le récepteur de la tyrosine kinase MET, exprimé à la surface des cellules épithéliales. L'activation du récepteur MET entraîne l'activation de la PI3K/AKT et des voies RAS/RAF/ERK. Le HGF est fondamental pendant l'embryogenèse. À l'âge adulte, le HGF est inactif, mais son expression se poursuit dans des cas spécifiques comme la régénération des tissus endommagés. Le HGF joue un rôle important dans la régulation du métabolisme du glucose dans le diabète de type 2 (DT2). Les taux locaux et plasmatiques de HGF sont élevés chez les patients résistants à l'insuline et compromettent la sensibilité à l'insuline. Néanmoins, la surexpression ou l'injection de HGF humain exogène chez les souris maintient la fonction de masse des cellules bêta et améliore la sensibilité à l'insuline. Le HGF est impliqué dans la régénération du foie. Le HGF favorise la prolifération des hépatocytes via MET, en modulant l'expression des régulateurs mitogéniques, et l'expression des chimiokines et des cytokines par les hépatocytes et les cellules inflammatoires résidentes. Le HGF constitue donc un médicament potentiel pour le traitement du DT2. En outre, l'effet régénérateur du foie du HGF peut être exploité pour le traitement de la stéatose hépatique non alcoolique (NAFLD) et des lésions hépatiques, où la capacité du foie à se régénérer est compromise. Cependant, l'application clinique du HGF présente plusieurs limitations pharmacocinétiques. C'est pourquoi nous avons conçu et produit un nouveau mimétique du HGF appelé K1K1. K1K1 est une protéine recombinante constituée d'un fragment modifié de HGF, et c'est un puissant agoniste MET puissant avec de meilleures propriétés pharmacocinétiques que HGF.L'objectif principal est de valider l'hypothèse selon laquelle K1K1 peut être utilisé comme thérapeutique mimétique du HGF pour 1) promouvoir la prolifération des hépatocytes pour la régénération du foie (NAFLD, lésions hépatiques) ; 2) améliorer la glycémie chez les souris DT2.Nous avons démontré l'effet mimétique du HGF de K1K1 sur des hépatocytes humains immortalisés (IHH). K1K1 active la signalisation MET dans les IHH, améliore la viabilité des IHH via le récepteur MET, protège contre la mort cellulaire induite par l'inhibiteur du récepteur MET SU11274. Comme le HGF, K1K1 induit l'expression des cyclines et des cytokines impliquées dans la prolifération des hépatocytes et la régénération du foie, telles que l'Interlekin 8 (IL8) et le Transforming Growth Factor Beta 1 (TGF-B1). De plus, K1K1 induit la sécrétion physiologique d'IL-8 et de TGF-B1 dans le surnageant des IHH. Chez les souris saines, K1K1 favorise la croissance de la masse hépatique.Pour étudier l'effet antidiabétique de K1K1 et son rôle contre la glucolipotoxicité dans les îlots des souris diabétiques, nous avons utilisé des souris db/db. Nous avons injecté quotidiennement K1K1 par voie intrapéritonéale pendant 3 semaines chez les souris diabétiques. Le traitement s'est révélé très efficace contre le diabète car K1K1 : 1) réduit l'hyperglycémie ; 2) n'a pas d'impact sur le poids pendant le traitement ; 3) réduit l'hyperinsulinémie à jeun en améliorant la sensibilité à l'insuline ; 4) améliore la tolérance au glucose et à l'insuline ; 5) améliore la sécrétion d'insuline en réponse au GSIS dans les îlots de souris diabétiques ex vivo. Nous avons également exploité une nouvelle stratégie thérapeutique consistant à combiner K1K1 avec le liraglutide (mimétique du glucagon-like peptide 1), qui a permis de renforcer encore tous les effets bénéfiques de K1K1. En conclusion, K1K1 est capable de mimer HGF au point d'en produire tous les effets biologiques en activant MET. K1K1 constitue donc un candidat médicament prometteur pour le traitement du DT2 et la régénération du foie en cas de NAFLD et de lésions hépatiques.</dcterms:abstract>
<dcterms:abstract xml:lang="en">Hepatocyte growth factor (HGF) is a human protein produced by stromal cells, which exerts several functions by binding and activating the MET tyrosine kinase receptor, expressed on the surface of epithelial cells. Activation of the MET receptor leads to activation of the PI3K/AKT and RAS/RAF/ERK pathways. HGF is essential during embryogenic processes. In adulthood, HGF is inactive, but its expression continues in specific cases such as the regeneration of damaged tissue. HGF plays an important role in the regulation of glucose metabolism in type 2 diabetes (T2D). Local and plasma HGF levels are elevated in insulin-resistant patients, thus compromising insulin sensitivity. Nevertheless, overexpression or injection of exogenous human HGF in mice maintains beta-cell mass function and improves insulin sensitivity. HGF is involved in liver regeneration. HGF promotes hepatocyte proliferation via MET, by modulating the expression of mitogenic regulators, and the expression of chemokines and cytokines by hepatocytes and resident inflammatory cells. HGF is therefore a potential drug for the treatment of T2D. In addition, the liver regenerative effect of HGF can be exploited for the treatment of non-alcoholic fatty liver disease (NAFLD) and liver injury, where the ability of the liver to regenerate is compromised. However, the clinical application of HGF is hampered by several pharmacokinetic limitations. For this reason, we designed and produced a novel HGF mimetic called K1K1. K1K1 is a recombinant protein consisting of a modified fragment of HGF, and is a potent MET agonist, as demonstrated in a wide range of in vitro and vivo studies, with better pharmacokinetic properties than HGF.The main objective is to validate the hypothesis that K1K1 can be used as an HGF mimetic therapeutic to 1) promote hepatocyte proliferation for liver regeneration (NAFLD, liver injury); 2) improve glycemia in T2D mice.We have demonstrated the HGF-mimetic effect of K1K1 on immortalized human hepatocytes (IHH). K1K1 activates MET signaling in IHHs, improves IHH viability via the MET receptor, protects against cell death induced by the MET receptor inhibitor SU11274. Like HGF, K1K1 induces expression of cyclins and cytokines involved in hepatocyte proliferation and liver regeneration, such as Interlekin 8 (IL8) and Transforming Growth Factor Beta 1 (TGF-B1). In addition, K1K1 induces physiological secretion of IL-8 and TGF-B1 in HHI supernatants. In healthy mice, K1K1 promotes the growth of liver mass.To study the anti-diabetic effect of K1K1 and its role against glucolipotoxicity in the islets of diabetic mice, we used db/db mice. We injected K1K1 daily intraperitoneally for 3 weeks in diabetic mice. The treatment proved highly effective against diabetes, as K1K1: 1) reduces hyperglycemia; 2) has no impact on body weight gain during treatment; 3) reduces fasting hyperinsulinemia by improving insulin sensitivity; 4) improves glucose and insulin tolerance; 5) improves insulin secretion in response to GSIS in islets of diabetic mice ex vivo. We also exploited a novel therapeutic strategy of combining K1K1 with liraglutide (a glucagon-like peptide 1 mimetic), which further enhanced all the beneficial effects of K1K1. In conclusion, K1K1 is capable of mimicking HGF to the point of producing all its biological effects by activating MET. K1K1 is therefore a promising drug candidate for the treatment of T2D, liver regeneration in NAFLD and other liver injuries.</dcterms:abstract>
