Cette thèse a pour double objectif de 1) mieux appréhender l'impact de la manière dont les individus s'adaptent à l'incertitude en médecine périnatale sur les expériences et les décisions et 2) examiner les fondements épistémologiques sur lesquels repose le travail interdisciplinaire entre sciences médicales et psychologiques. La thèse s'articule autour de trois études scientifiques ; la première emploie une approche phénoménologique interprétative afin d'explorer la façon dont des parents ayant vécu une décision de soins palliatifs pour leur nouveau-né construisent du sens autour de leur expérience. La deuxième utilise la technique de modélisation par équations structurelles en moindres carrés partiels (PLS-SEM) pour examiner le rôle médiateur de l'intolérance à l'incertitude dans la relation entre perfectionnisme parental et dépression postnatale chez des mères tout venantes. La troisième utilise une méthodologie par vignette clinique pour explorer l'hypothèse que, en néonatalogie, le stress ressenti par les soignants face à l'incertitude influence leur perception de l'acceptabilité d'initier ou de poursuivre une prise en charge réanimatoire dans un cas fictif d'extrême prématurité. Les trois études s'intéressent au phénomène de l'adaptation face à l'incertitude en périnatalité à travers des approches méthodologiques différentes. Elles montrent que le rapport à l'incertitude façonne les expériences et les décisions tant chez les parents que chez les soignants. Des recommandations sont proposées pour améliorer les pratiques cliniques, la formation des soignants et l'accompagnement des familles. Par ailleurs, des propositions visant à faciliter la collaboration interdisciplinaire entre psychologie et médecine périnatale sont avancées, notamment à travers une meilleure compréhension des postulats épistémologiques de chaque discipline. In the field of perinatal care, the way in which parents and healthcare providers react to uncertainty impacts their experiences and perception of the choices they are faced with. The aim of this thesis is twofold: 1) to gain a better understanding of the impact of responses to uncertainty on parents' and healthcare professionals' experiences and decisions, and 2) to examine the epistemological foundations underpinning interdisciplinary research work involving medicine and psychology. The thesis is centred upon three scientific studies; the first employs an interpretative phenomenological approach to explore how parents who have lived through a palliative care decision for their newborn construct meaning around their experience. The second uses Partial Least Squares Structural Equation Modelling (PLS-SEM) to examine the mediating role of intolerance of uncertainty in the relationship between parental perfectionism and postnatal depression in mothers during the first year postpartum. The third uses clinical vignette methodology to explore the hypothesis that, in neonatology, the stress experienced by healthcare providers when faced with uncertainty influences their perception of the reasonableness of initiating or pursuing resuscitative management in a fictitious case of prematurity at the edge of viability. All three studies address the topic of adapting to uncertainty in the perinatal period through different methodological approaches. They show that the way in which parents and healthcare providers respond to uncertainty shapes their experiences and decisions. Recommendations for improving clinical practice, professional training and family support are presented. Proposals for fostering interdisciplinary collaboration between psychology and perinatal medicine are also discussed, namely through a better understanding of the epistemological foundations of each discipline.
