Les données concernant la mortalité et le pronostic fonctionnel au long cours des HIC sont limitées. Les objectifs principaux de cette thèse étaient d’évaluer chez les survivants à une HIC: (i) le taux de mortalité et la fréquence du déclin fonctionnel au long cours; (ii) les prédicteurs cliniques et radiologiques de mortalité et de déclin fonctionnel au long cours; (iii) l’impact des événements cliniques majeurs sur la mortalité et le déclin fonctionnel au cours du suivi.Méthodes : Nous avons inclus tous les patients admis au CHU de Lille pour une HIC spontanée entre 2004 et 2009, recrutés de façon prospective et consécutive dans l’étude observationnelle PITCH (Prognosis of IntraCerebral Haemorrhage). Résultats : Le taux cumulé de survie à 10 ans de l’HIC était de 38%. Vingt-trois % des survivants à 6 mois présentaient un déclin fonctionnel pendant une médiane de 9 ans de suivi. En analyses multivariées, l’âge, le sexe masculin, la gravité de l’HIC et la présence d’une démence ou de nouveaux accidents vasculaires cérébraux (AVC) pendant le suivi étaient indépendamment associés à la mortalité à long terme. Parmi les marqueurs IRM, seule l’atrophie cérébrale était associée à la mortalité au long cours. Le déclin fonctionnel était indépendamment associé à l’âge, le volume de l’hémorragie, l’état fonctionnel à 6 mois, la survenue de démence et d’AVC (ischémique et hémorragique) au cours du suivi.Conclusions : Les résultats de ce travail ont un impact clinique important. Ils soulignent la pertinence d’un suivi au long cours chez les patients ayant présenté une HIC. Ils apportent des informations nouvelles sur le risque de mortalité et de déclin fonctionnel de même que sur les prédicteurs qui leur sont associés. Background: Spontaneous (non-traumatic) intracerebral hemorrhage (ICH) is the most dramatic type of stroke being responsible for the majority of short-term mortality and stroke-related disability. The main objective of this thesis was to determine among ICH survivors: (i) rate of mortality and frequency of functional decline after ICH; (ii) clinical and radiological predictors of long-term mortality and functional decline; (iii) the influence of major clinical events during follow-up on long-term mortality and functional decline.Methods: We included all consecutive ICH patients admitted at the Lille University Hospital for spontaneous ICH between 2004 and 2009 prospectively enrolled in the observational PITCH (Prognosis of IntraCerebral Haemorrhage) study.Results: In ICH survivors, the cumulative survival rate at 10 years was 38 %. During a median follow-up of 9 years, 23 % of patients experienced a functional decline. In multivariable analysis, variables independently associated with long-term mortality were age, male gender, prestroke dependency, ICH clinical severity at presentation, occurrence of any stroke, and dementia during follow-up. Among MRI markers, only cerebral atrophy was independently associated with long-term mortality. Functional decline after ICH was independently associated with age, ICH volume, functional status at 6 months, occurrence of major clinical event during follow-up.Conclusion: The results of this work have an important clinical impact. They underline the relevance of long-term follow-up in patients with ICH. They provide new insights on the risk of mortality and functional decline as well as on their associated predictors.
