<dcterms:abstract xml:lang="fr">Les premières années de la vie sont marquées par un développement important des poumons et du système immunitaire, immatures à la naissance. Les défenses du poumon font intervenir en premier lieu, l’immunité innée qui, en période néonatale, contribue au déséquilibre de la réponse adaptative avec un biais inhérent en défaveur d’une immunité anti-infectieuse qui facilite une réponse allergique. La théorie hygiéniste suggère que le développement d’un asthme est, au moins en partie, attribuable à une dérégulation immunitaire, par manque d'exposition à certains microorganismes. L'asthme, première maladie chronique de l’enfant, est favorisé par des facteurs de risque génétiques, épigénétiques et environnementaux. Différents profils d’évolution ont été décrits et les facteurs susceptibles d’influencer l’histoire naturelle sont multiples. Sur le plan lésionnel, le remodelage apparait précocement, probablement entre 3 et 6 ans. L’âge préscolaire constitue donc une période cruciale pour caractériser les formes sévères et limiter des lésions irréversibles. Plusieurs approches permettent de catégoriser les patients asthmatiques, soit par l’identification de caractéristiques cliniques spécifiques, définissant un "phénotype", soit par l’identification de processus physiopathologiques à l’origine de l’inflammation, définissant un "endotype". Les exacerbations constituent un événement particulier, fréquent chez le jeune enfant, propice à l’étude de la réponse inflammatoire. Hypothèse : L’hétérogénéité des phénotypes d’asthme de l’enfant est la résultante de la variabilité interindividuelle de la réponse immune de la muqueuse bronchique, en partie prédéterminée génétiquement, et en partie influencée par les expositions environnementales, microbiennes en particulier. Mieux comprendre l’asthme de l’enfant en tenant compte des mécanismes pourrait permettre d’améliorer le traitement, pour prévenir l’altération fonctionnelle et les lésions anatomiques. Objectifs et principaux résultats du travail de thèse :1/ Décrire les facteurs associés à la sévérité dans l’asthme de l’enfant. L’analyse des données de la cohorte nationale CobraPed suggère que les facteurs influençant la sévérité de l'asthme différent selon l'âge : elle est associée à l’exposition au tabac et/ou aux moisissures à l’âge préscolaire, alors qu’elle est reliée aux maladies atopiques à l’âge scolaire.2/ Corréler les endotypes et les phénotypes à l’âge préscolaire. L’analyse de la réponse immunitaire au cours d’infections respiratoires (VIRASTHMA 2) a mis en évidence chez les enfants les plus sévères, exacerbateurs fréquents, des niveaux systémiques plus faibles de cytokines Th1 et Th2, pro et anti-inflammatoires et des réponses antivirales moins marquées pendant une exacerbation sévère viro-induite.3/ Corréler les endotypes et les phénotypes d’évolution. L’analyse de la réponse immunitaire en parallèle du profil d’évolution clinique à un an chez des enfants d’âge scolaire (VIRASTHMA) n’a pas mis en évidence de relation entre le profil cytokinique au moment de l'exacerbation et le risque futur de mauvais contrôle de l'asthme et/ou de récurrence des exacerbations.4/ Générer des hypothèses sur les interactions entre microbiote et environnement atopique. L’analyse du métabolome plasmatique par profilage non-ciblé chez les enfants des cohortes VIRASTHMA 1 et 2 a permis d’identifier un défaut d’acides biliaires secondaires associé à l’atopie, suggérant que ces métabolites, connus pour être générés par le microbiote intestinal et avoir des fonctions immunomodulatrices, pourraient jouer un rôle clé dans la tolérance pendant l'enfance [...]</dcterms:abstract>
<dcterms:abstract xml:lang="en">During the first years of life, the lung and airway grows and the immune system, immature at birth, develops. Innate immunity, as the first line of host defense, plays a key role and contributes to the imbalance of the neonatal adaptive response, characterized by an inherent bias against anti-infectious immunity that facilitates an allergic response. The hygiene hypothesis suggests that the development of asthma is, in part at least, attributable to immune dysregulation due to lack of exposure to certain microorganisms.Asthma, the most frequent chronic disease in children, is favored by genetic, epigenetic, and environmental risk factors. Different evolutionary patterns have been described, and the factors likely to influence the natural history of asthma are multiple. In terms of structural damage, remodeling appears early in the natural history of asthma, probably between 3 and 6 years of age. The preschool age could therefore constitute a window of opportunity to characterize severe asthma and to prevent irreversible lesions. Several approaches have made it possible to categorize asthma patients, either by identifying their specific clinical characteristics, named “phenotype”, or by the underlying pathophysiological process leading to inflammation, named “endotype”. Exacerbations are a particular event, frequent in young children, which is conducive to the study of the inflammatory response.Hypothesis: The heterogeneity of asthma phenotypes in children is the result of inter-individual variability of the bronchial mucosa immune response, partly determined by genetic factors, and partly influenced by environmental, among which microbial exposures. A better understanding of childhood asthma, taking into account its mechanisms, could lead to improved treatment to prevent functional impairment and anatomical damage. Objectives and main results of the PhD thesis:1/ To describe factors associated with severity in asthmatic children.The analysis of data from the French national CobraPed cohort suggests that factors influencing asthma severity may differ according to age: exposure to tobacco and/or molds is associated with severity at preschool age, whereas atopic diseases are associated with severity at school age.2/ To correlate endotypes and phenotypes at preschool age. The analysis of the immune response during respiratory infections (VIRASTHMA 2) showed in the most severe children, prone to exacerbations, lower systemic levels of Th1 and Th2, pro- and anti-inflammatory cytokines and antiviral responses during a severe virus-induced exacerbation.3/ To correlate endotypes and evolutionary phenotypes. The analysis of the immune response in parallel with the clinical evolutionary profile at one year in school-age children (VIRASTHMA) did not display a relationship between the cytokine profile at the time of exacerbation and future risk of poor asthma control and/or recurrence of exacerbations.4/ To generate hypotheses on the interactions between microbiota and atopy. The analysis of untargeted profiling of the plasma metabolome from children from the VIRASTHMA 1 and 2 cohorts allowed to identify a defect in secondary bile acids associated with atopy, suggesting that these metabolites, known to be generated by the intestinal microbiota and with immunomodulatory functions, could play a key role in tolerance during childhood. In conclusion, this thesis confirms the utility of phenotyping asthmatic children and describing their environment. Prone to exacerbation phenotype could correspond to a distinct endotype, characterized by a defect of the anti-infectious response. The microbiota also plays a key role in the development of atopy and the perpetuation of asthma.</dcterms:abstract>
