Contexte: Bien que les chirurgies bariatriques soient reconnues aujourd'hui comme le meilleur traitement pour lutter contre l’obésité, ces interventions peuvent également engendrer des conséquences physiques et psychologiques délétères sur le long terme L’objectif de cette thèse est d’évaluer l’efficacité d’un programme innovant d’intervention de Thérapies Émotionnelles Cognitives et Comportementales (TECC), s’appuyant sur la Théorie de l’Autodétermination (TAD, Deci & Ryan, 1985, 2002, 2017). A partir d’une recension de la littérature examinant les stratégies en TECC les plus efficientes, notre programme s’est enrichi de l’apport de la TAD afin de permettre une meilleure satisfaction des besoins d’autonomie, de compétence et d’affiliation sociale dans le cadre de l’accompagnement des patients. Méthode : 61 candidats à une chirurgie bariatrique ont participé au programme (groupe témoin, n=35 ; groupe expérimental, n=26). Notre programme TECC-TAD, proposé au groupe expérimental, a ciblé différents facteurs impliqués dans les échecs chirurgicaux : troubles du comportement alimentaire, syndrôme anxio-dépressif, degré de motivation autodéterminée, insatisfaction corporelle. Résultats : Les analyses préliminaires (ANCOVA) révèlent pour les participants du groupe expérimental un effet statistiquement significatif de notre programme sur l’amélioration des : TCA, anxiété, dépression, estime et image de soi, qualité de vie mentale, motivation autodéterminée et satisfaction des 3 besoins psychologiques, en comparaison avec le groupe contrôle. Discussion : Ces premiers résultats sont prometteurs et l’évaluation sur du long terme de ce programme interventionnel permettra d’enrichir notamment les recommandations internationales dans le domaine des chirurgies bariatriques et ainsi contribuer au bien-être des patients.
Context: Although bariatric surgery is now recognised as the most effective treatment for obese patients, it appears that these operations can also have harmful physical and psychological consequences in the long term. The aim of this thesis is to evaluate the effectiveness of an innovative programme involving Cognitive Emotional Behavioral Therapy (CEBT), based on Self-Determination Theory (SDT, Deci& Ryan, 1985, 2002, 2017). Based on the recension of publications examining the most efficient CEBT strategies, our programme has been enriched by the contribution of SDT in order to better fulfil the needs for autonomy, competence and social affiliation as part of patient support. Method: 61 candidates for bariatric surgery took part in the programme (control group, n=35; experimental group, n=26). Our CEBT-SDT programme offered to the experimental group targeted different factors involved in surgical failures: eating disorders, anxio-depressive disorders, level of self-determined motivation, body dissatisfaction. Results: Preliminary analyses (ANCOVA) reveal a statistically significant effect within the experimental group, with improvements in following areas as compared with the control group: eating disorders, anxiety, depression, self-esteem and self-image, mental health, self-determined motivation, fulfilment of the three psychological needs. Discussion: These initial results are promising and the long-term evaluation of this intervention programme will make it possible to enrich the international recommendations in the field of bariatric surgery and thereby contribute to patients’ well-being.
