La reinstillation digestive (RD) est un procédé qui consiste à recueillir le chyme d'une stomie d'amont pour le transférer en aval chez des patients porteurs de double enterostomie chez des patients à haut débit stomiaux en attente d'un rétablissement de continuité chirurgical. La RD peut s'appliquer dans les fistules grêles haute ou les iléostomies latérales. Ce by-pass extra corporel apporte un bénéfice nutritionnel, permet de sevrer le patient de l'alimentation parentérale et d'éviter les complications associées, entretient le microbiote intestinal d'aval. Cependant, la RD souffre encore d'un manque de matériel dédié à sa réalisation, entrainant un manque d'enthousiasme pour cette procédure jugée contraignante tant pour le personnel soignant que pour les patients. Nous avons développé un dispositif de réinstillation digestive appellé EXCEP (Extra Corporeal Enteral Prosthesis) permettant de transvaser le chyme d'amont vers le segment d'aval avec les avantages d'être ergonomique, ambulatoire et efficient. Le but de ce travail de thèse est d'évaluer et d'optimiser le dispositif EXCEP en vue de l'utilisation chez l'homme.
The digestive reinfusion (DR) is a proccess which collects the chyme from the upstream stoma and bring it to the downstream one, in patients with two enterostomies with high output, waiting for continuity recovery. The DR can be applied for enteral fistula or lopp ileostomy. This external by-pass brings a nutritional benefit, may wean patient from parenteral nutrition and avoid associated complications, and maintains upstream intestinal microbiota. However, There is few material specifically dedicated to the DR procedure. This fact causes a lack of enthusiasm for the DR, considered as too heavy for the patient and the nursing staff. We developped a DR device called EXCEP (EXtra Corporeal Enteral Prosthesis) which transfer the chyme from the proximal stoma to the distal one with ergonomic, ambulatory and efficient advantages. The aim of this work is to evaluate and to optimize our device EXCEP for use in humans.
