<dcterms:abstract xml:lang="fr">L’utilisation quotidienne des solutions glucosées injectables (SGI) au sein des établissements de santé, impose une sureté de leur administration, que ce soit en tant que véhicule d’administration, solution d’hydratation, ou composant des solutions parentérales ou de nutrition parentérale. L’administration par voie parentérale impose une stérilité des solutions. La Pharmacopée Européenne recommande la stérilisation par la chaleur humide, méthode connue pour favoriser la formation de produits de dégradation du glucose (PDG), précurseur des AGEs (Advanced Glycation End products). Il a été montré que de hautes concentrations en PDG et AGEs ont un impact sur l’homéostasie cellulaire et la mise en place de pathologies, notamment au sein du système cardio-vasculaire et rénal. Afin de limiter l’administration de PDG aux patients, il est important de déterminer les facteurs influençant leur formation et de tenter de les maîtriser, ce qui est l’objectif de ce travail.La première partie a été de développer et de valider une méthode de dosage de deux PDG, le 5-hydoxyméthyl-2-furladéhylde (5-HMF) et le 2-furaldéhyde (2-FA). Après validation de la méthode de dosage par CLHP-UV, 84 SGI ont été étudiées, provenant de 5 fabricants, avec 8 concentrations en glucose, 6 volumes, 3 types de contenants et 6 matériaux différents. Cette analyse du marché français montre une influence du temps de stockage, de la quantité initiale en glucose, de la perméabilité à l’oxygène du contenant sur la vitesse de formation de ces deux PDG. Les matériaux des contenants et les fournisseurs ont été classés en fonction de leur impact. Le verre et le polypropylène semblent limiter la formation des PDG contrairement au bicouche polyéthylène/polypropylène.La dernière partie consiste en l’étude de la stérilisation des SGI à 5%, 10%, 30% et 50%, stérilisées par filtration et par la chaleur humide (10 cycles de stérilisation). Quatre paramètres de stérilisation ont été étudiés : la température (111, 11, 121 et 134°C), la durée (3, 9, 15, 20, 30, 90 et 200 min), l’effet stérilisant F0 (3, 9, 9,5, 15, 20, 30 et 60 min) et l’autoclave (L et B). La filtration ne génère pas plus de PDG que l’absence de stérilisation, alors que la stérilisation à chaud montre une augmentation de leur production quel que soit le cycle choisi. La vitesse de formation des PDG est corrélée à la température du cycle, à l’effet stérilisant F0 et à la concentration en glucose. L’ensemble de ces résultats a permis de définir les premières conditions optimales de stérilisation par la chaleur, recommandant de se placer à la température la plus élevée pour un F0 donné, afin d’avoir la durée de stérilisation la plus courte possible.En conclusion, afin de limiter le risque d’exposition des patients aux PDG, différentes mesures peuvent être appliquées : utiliser de faibles concentrations de glucose, dans des contenants en verre de faible volume, ayant été stérilisés par filtration stérilisante ou par un autoclave à haute température durant un cycle court.</dcterms:abstract>
<dcterms:abstract xml:lang="en">The daily use of sterile glucose solutions for infusion (GSI) in hospital imposes a safety of their administration. They are used as diluents for injectable drugs, as hydration solutions or as well as for parenteral solutions and parenteral nutrition. The intravenous administration requires sterilization. However, the moist heat sterilization process, recommended by European Pharmacopoeia, generates glucose degradation products (GDP). GPDs are highly reactive precursors of Advanced Glycation End products (AGEs) which high levels could have an impact on cell homeostasis and pathogenesis, especially on cardiovascular and renal systems. In order to limit the administration of PDG to patients, it is important to determine the factors influencing their formation and try to control them, which was the objective of work.The first part was to develop and validate an analytical method tow GDP, 5-hydoxymethyl-2-furladehylde (5-HMF) and 2-furaldehyde (2-FA). After validation of analytical method by HPLC-UV, 84 GSI were studied, coming from 5 suppliers at 8 concentrations, 6 volumes, 3 types of containers and 6 different materials.This analysis of the French market determinate a significant influence of storage time, initial glucose amount, and permeability to oxygen of containers on the GDPs formation rates.The influence of container material and suppliers could be classified according to their ability to limit GDP generation. For instance, the GDP formation rate is lower with both polypropylen and glass.The second part consists in the study of sterilization process on GSI at 5%, 10%, 30% and 50% and sterilized by filtration or moist heat (10 cycles). Four sterilization parameters were analysed: temperature (111, 11, 121 or 134°C), time (3, 9, 15, 20, 30, 90 or 200 m), sterilization effect F0 (3, 9, 9, 5, 15, 20, 30 or 60 min) and autoclave (L or B). The filtration process did not generate more GDP than without sterilization, whereas moist heat sterilization improves their production in all conditions. The GDP formation rate was correlated with cycle temperature, sterilization effect F0 and glucose concentration. These data have enabled to define optimal heat moist sterilization conditions, which recommended the highest temperature for an F0 in order to have the smallest sterilization length.To conclude, in aim to limit GDP exposition risk for patients, numerous measures could be applied: use low glucose concentration in glass small volume container, sterilized with high temperature autoclave short cycles.</dcterms:abstract>
